Insurance Coverage
I am an out of network provider for some insurance companies. What this means is that I am not contracted with any insurance companies. Some plans allow patients to see providers they are not contracted with and will pay a portion of the fees. It is best to call your particular insurance company to see what their policy is in this case. If you choose to see me I will provide you with a “superbill” containing the billing and diagnostic codes to submit to your insurance. You will pay me at the time of service and work with your insurance for reimbursement.
Cancellation Policy
I require at least 24 hours notice to cancel an appointment without a fee. Sessions cancelled in less than 24 hours will be charged the full fee. I will waive the fee in the case of an emergency and also offer a 1x cancellation due to illness or other unforeseen events. You will have access to me via email, text and phone.
